Screw backs are more secure. Push backs are more convenient. For diamond studs you intend to wear every day, choose screw backs.
How each works
A screw back has a threaded post; the back turns onto it and locks in place, and cannot come off without being unscrewed. A push back, also called a friction or butterfly back, slides onto a smooth post and is held there by friction against a small notch.
Why push backs loosen
Friction weakens with use. Each time a push back is removed and replaced, the grip on the post eases slightly, and over months the back can loosen enough to slip off. This gradual loosening is the most common way a stud is lost. A threaded post does not loosen through wear in the same way.

Which to choose
Choose screw backs if the earrings will stay in most of the time, if you sleep in them, if you travel with them, or if they carry a carat weight you could not easily replace. Choose push backs if you change earrings often and always take them out at night.
A note for children and first earrings
Screw backs are often preferred for children's earrings for the same reason ā they stay put. Check the post length suits the earlobe, and follow the piercer's advice on newly pierced ears.
Looking after them
Push backs should be checked from time to time: hold the stud and gently try to pull the back off. If it slides off easily, the grip has worn and the back should be replaced. Screw backs only need checking that they are fully tightened, and an occasional clean of the threads.
